Case study · Therapy clinic

Eleven years in business. One profitable month changed how the owner sees it.

A well-loved clinic had more demand than it could convert and had never made real money. A few months later, it did.

36% → 53%Inquiries that became clients, in one month
27% → 17%Families lost because no one followed up
Month 1Of real profit, after 11 years

The situation

The owner had built a respected clinic over eleven years. Families sought it out, referrals came in, and the clinical work was excellent. But financially, the business had never done better than break even, and many months it lost money. The owner was carrying all of it and was, in her own words, exhausted.

On paper this looked like a revenue problem. It wasn't. Demand was there. It was leaking out between the first phone call and the first session.

What was really going on

Like a lot of owners who are great at the work itself, she ran the business on care and instinct, not numbers. No one was measuring intake. When we looked, more than a quarter of new inquiries were lost simply because no one got back to the family in time. Each one was a family that wanted help and a client the clinic had already paid to attract.

That gap wasn't a staffing failure. It was what happens when the owner is the only person holding the whole picture, and there's no system to hold it with her.

What we changed

  • Intake became measurable. Every inquiry tracked by source, outcome, and reason lost, reviewed monthly.
  • Follow-up became a process, not a hope. Clear ownership and a timeline for getting back to every family.
  • The best referral sources got attention. Physician referrals grew and became one of the highest-converting channels.
  • Profit became a decision. The owner started looking at the numbers every month, with a target, instead of hoping something was left over.

What happened

Within one month, the share of inquiries that became clients rose from 36% to 53%. Families lost to no follow-up fell from 27% to 17%. A few months into the work, the clinic posted its first truly profitable month in eleven years.

The change I'm proudest of is less visible. The owner now produces her own intake report, reads it, and acts on it. She sees her business differently, and that's what keeps the results from fading.

“For the first time in 11 years we are truly profitable. Thank you for helping me turn this ship around in just a few short months.”
